Admissions

Candidates for admission may be referred by the Department of Family and Children Services, Department of Juvenile Justice, Multi-Agency Alliance for Children, and the following community service boards: McIntosh Trail, New Horizons, Mid Flint and Pathways Center for Behavioral and Developmental Growth. Juvenile Court Judges, physicians, psychiatric hospitals, schools, other appropriate social service agencies, parents, discharge planners and case managers may also make referrals.

Referrals should include:
Psychological Evaluation (within 1 year)
Psychosexual Evaluation (if applicable)
Social Summary
Medical Records
Education records (including current IEP)
Order for Custody
Commitment Order
Petition
Proof of Medicaid or Other Insurance Coverage
Criminal Record
Police Report(s)
Witness/Victim Statements
Other Additional Information as Required

Any information disclosed at Twin Cedars concerning clients including their identity is confidential and protected by Federal and State Law. These laws and Twin Cedars' policy prohibit any disclosures of confidential information without the consent of the person whom the information pertains, with the exception of statutory duty to report child abuse.
